For hospital teams & social workers

Hospital to Home transitions

We support people with disability transitioning from hospital back into the community with their NDIS plan. Our team works alongside hospital discharge planners, social workers, and allied health professionals to ensure safe, supported transitions.

Careable supports people returning home after a hospital stay across Melbourne's north and north-east. Our supported living homes are in Heidelberg Heights and Macleod, five and seven minutes from the Austin Hospital.

How the referral process works

  1. Send a referral

    Complete our professional referral form online or call us. Include relevant discharge information and support needs.

  2. We review the referral

    Our team reviews the referral and assesses our capacity to support the participant's needs and NDIS plan.

  3. We make contact

    We acknowledge every referral within one business day and give you a clear answer on capacity within two business days — often the same day. We then contact the participant and their family to discuss supports and confirm next steps.

  4. Supports begin

    Once confirmed, we work with the participant and their team to put supports in place and enable a safe transition home.

Psychosocial disability

Psychosocial disability support

We support people living with psychosocial disability — disability that arises from a mental health condition that impacts a person's daily life.

Our psychosocial disability supports can assist people who have previously spent time in clinical or recovery settings such as Prevention and Recovery Care (PARC) services and Secure Extended Care Units (SECU), as they build everyday living skills and reconnect with their community.

We work alongside community mental health teams, psychiatrists, and clinical case managers to ensure a safe, supported recovery environment.
