Transitioning into a SIL home from hospital involves a structured process to ensure the right supports are in place from the outset. We follow a five-step discharge planning and execution process covering early referral, multidisciplinary planning, environment and staffing readiness, day-of-discharge execution and post-discharge monitoring.
