What this is
This service supports people with psychosocial disability who are stepping down from bed-based mental health services — including Prevention and Recovery Care (PARC) and Secure Extended Care Unit (SECU) settings — into community living or Supported Independent Living.
A successful transition from a mental health setting into stable housing requires careful planning, close coordination with clinical teams, and the right supports in place from day one. Careable brings a recovery-oriented approach to every step of that process.

How we support step-down
Recovery-oriented community support
Support built around the participant's own goals, delivered with patience and consistency to promote recovery and independence.
Coordination with clinical teams
With your consent, we can keep in touch with the people already supporting you, so your supports work together as you settle back home.
SIL and intensive in-home options
Participants can transition into Supported Independent Living or receive intensive in-home support, depending on their needs and NDIS plan.
Focus on stable housing
We aim to provide stable, consistent housing and support to reduce the risk of readmission and support long-term recovery.
Behaviour support plan implementation
We are registered to implement behaviour support plans for participants with behaviours of concern.
Post-transition review
We monitor closely in the early weeks and stay in contact with the care team to identify and respond to any emerging needs.
Collaboration
Working with mental health services
Careable can provide support to people moving on from clinical mental health settings — including Prevention and Recovery Care (PARC) services and Secure Extended Care Units (SECU) — as they transition back to living in the community. Early engagement helps us understand the participant's clinical history, risk factors and recovery goals before discharge day.
We recognise that transitions from intensive mental health settings can be complex and that the period immediately after discharge requires careful monitoring and responsive support. Our team is experienced in working alongside clinical professionals and adjusting supports as circumstances change.
Referrals are accepted from support coordinators, clinical teams and families at any stage of admission. We aim to provide honest capacity updates and work transparently with all parties throughout the process.

Our experience
Careable has experience supporting participants with psychosocial disability transitioning from acute and sub-acute mental health settings into Supported Independent Living, including complex cases where an earlier transition had broken down. Several participants have been living stably in our homes for multiple years.
