What is Support Coordination?
Support Coordination is a vital NDIS service that helps participants understand and implement their NDIS plans. Our experienced Support Coordinators work closely with individuals to build their capacity, increase independence, and connect with the most suitable service providers.
Types of Support Coordination We Offer
1. Support Connection
This is a short-term support designed to assist participants in understanding their NDIS plans and connecting with appropriate services. Our Support Connection services help individuals:
- Understand their NDIS funding and how to use it effectively
- Identify service providers that align with their needs and preferences
- Develop self-advocacy skills to manage their own supports in the future
2. Support Coordination (Level 2)
This level of support is ideal for participants who require ongoing assistance to coordinate their services. Our Support Coordinators will:
- Help develop a tailored support plan
- Find and engage with the best service providers
- Ensure services align with personal goals and NDIS plan funding
- Resolve service-related issues and liaise with the NDIS when needed
3. Specialist Support Coordination (Level 3)
For participants with complex needs, our Specialist Support Coordinators provide a higher level of support, working with multiple providers and managing crisis situations. This includes:
- Coordinating multiple supports for individuals with high or complex needs
