About this service

What is NDIS community nursing?

Community nursing is registered nursing care provided in your home or community setting. It is funded under the NDIS for participants with clinical health needs as part of their disability support. A registered nurse visits you at home or in your community to deliver skilled clinical care tailored to your health and disability needs.

  • Clinical monitoringOngoing monitoring of health conditions, vital signs and clinical indicators.
  • Coordination with GPs and allied healthWorking with your GP, allied health team and other providers to ensure coordinated care.
  • Emergency health response planningDeveloping emergency health response plans to keep you safe at home.

High intensity support

Careable is registered with the NDIS Commission for high intensity daily personal activities and audited against the NDIS Practice Standards for this work. We provide urinary catheter care under a nurse-assessed care plan:

  • nursing assessment and high intensity risk assessment before support starts
  • support from workers individually assessed as competent
  • daily fluid intake and output monitoring, and a personal emergency plan

How it works

  2. We review clinical needsOur clinical team reviews the participant's health and disability support needs.
  3. We assess capacityWe confirm we can meet the clinical needs and arrange an initial nursing assessment.
  4. Nursing support beginsA registered nurse begins delivering nursing care in the participant's home or community setting.
